If you operated a vehicle in Illinois under Lily Transportation, LLC’s federal motor carrier authority that were equipped with a Samsara dash-camera between July 10, 2019, and the date of preliminary approval without providing prior written consent, then you may be eligible to receive Settlement Benefits.

To qualify for Settlement Benefits, you must complete a Claim Form submitted online or postmarked on or before October 16, 2025.

To start your online claim, you can find your Notice ID & PIN printed on the Notice that was sent to you. If you are unable to locate your ID & PIN, please contact the Settlement Administrator at (833) 360-6862. Please request your Notice ID & PIN by providing the Class Member’s full name and mailing address.

Claim forms must be submitted or postmarked by October 16, 2025. Your failure to submit a timely claim form will result in you forfeiting any payment and/or benefits for which you may be eligible under the Settlement.
